It appears the HSE have gone ahead and withdrawn the Management ACOP The HSE Books entry for L21 now states: The Management of Health and Safety at Work Approved Code of Practice (ACOP – L21) has been withdrawn and is no longer available

Neither the ACOP nor the regulations require the recording of significant findings to be in any particular format Removing the ACOP does not change this, in my view It is only when H S bods overload the term 'risk assessment' as meaning some particular likelihood x severity approach that this requirement materialises

The Workplace, Health Safety and Welfare Regulations - ACOP to Regulation 13 states: Fixed ladders at an angle of less than 15 degrees to the vertical (a pitch of more than 75 degrees) which are more than 2 5 m high should where possible be fitted with suitable safety hoops or permanently fixed fall arrest systems

Returning to the original question, I think the ACOP paragraphs 256 to 264 actually does a pretty good job of defining what should be in the file Your entitled to disagree, and I am aware of what is stated in the acop and what the HSE considers should be in a H S file however every client I have ever worked for has requested this info be included
