There is not paperwork that goes into you IPERMS However to have it uploaded onto your ERB ORB SRB you need to provide the servicing personnel center with some documentation This can be either a copy of the DOD award citation (which lists the eligible dates), or a copy of the DA General Order which will also list the unit and dates

So I’m sure, I’m not only person that struggles with keeping records in my file and keeping it updated but why do files continuously disappear from iperms, multiple things get removed from the srb erb and things like last ncoer dates get changed to the wrong dates?
