An issue with Safe Mode is that it is a diagnostic and doesn't load all the drivers needed for hardware so any adjustment can be lost when restarting in Normal Mode I had a similar issue over 20 years ago, older hardware is not always supported by newer software

Only one HDMI port is capable of 60Hz, so you could try the other HDMI port or the DP port and see if it makes a difference I tried the other HDMI port, problem is still there And I don't have a DP cable right now
