I assume that you mean that it's a Form 1099-R As you discovered, a Form 1099-R that includes code B in box 7 is not permitted to have the IRA SEP SIMPLE box marked because code B means that the distribution is not from any kind of IRA You should try to escalate this with the payer to obtain a corrected form

What you need to do is to include 2 copies of form 5695 with your return, one for home A and one for home B Put the address of home A on form A line 21c, and the AC credit on line 22a-b, and put the address for home B on line 21c of form B, and the AC credit on line 22
